How the Plan Works

We provide you and your family with comprehensive and flexible medical insurance options. Both plans give you access to one of the country’s largest provider networks, UHC Choice Plus, and you won’t have to worry about referrals.

FranklinCovey offers a Copay Plan and High Deductible Health Plan (HDHP). Your in-network preventive care and virtual care is covered at 100% with no copays, coinsurance or deductibles. Benefits will be covered under the preventive care if services or supplies are in accordance with age limits and frequency guidelines.

New for 2026: The Surest Copay Plan is a new health plan offering through the United Healthcare insurance company, replacing the UHC $1,000 Deductible PPO. Click here for some key differences between the 2026 Surest Copay Plan and 2025 UHC $1,000 Deductible PPO.

This medical offering has no deductible. It is a copay-only plan design designed to put you in the driver’s seat so you can search and find the care you need, at prices you can see before making an appointment or receiving treatment. Copays are generally lower for providers or locations based on quality, efficiency, and effectiveness measures. The Surest plan uses the same UHC network.

Your Deductible

The deductible is the amount you pay each year for eligible in-network and out-of-network charges before the plan begins to pay.

Note that the Surest Copay Plan does not have a deductible of any kind. The plan begins to cover qualified expenses immediately at a set copay/fixed amount of money.

Your Coverage

Once your deductible is met, you and the plan share the cost of covered medical and pharmacy expenses with coinsurance.

Coinsurance means plan will pay a percentage of each eligible expense, and you will pay the rest.

Copay means you pay a flat dollar amount depending on the visit/service; the plan pays the rest.

Out-of-Pocket Maximums

When you reach your out-of-pocket maximum, the plan pays 100% of covered medical and pharmacy expenses for the rest of the plan year. Your deductible coinsurance, and any copays apply toward the out-of-pocket maximum.

ID Cards

You will receive physical ID cards in the mail a few weeks after your enrollment is completed in the ADP system. Once your information is passed over to UHC or Surest by ADP, you will have immediate access to your member ID cards and proof of coverage anytime via the UHC or Surest mobile app. View your ID cards on your mobile device to share them with your providers.

Pharmacy Benefits

The Prescription Drug List (PDL) is a list of commonly prescribed medications covered by the plan. Medications are placed into tiers that represent the cost you pay out of pocket. Choosing medications in the lower tiers may save you money.

OptumRx is your plan’s pharmacy care services manager. OptumRx is included with your medical plan offerings (regardless if you are enrolled in the Surest Copay Plan or UHC $2,400 HDHP). OptumRx offers many convenient ways to fill and save on medications. You can access thousands of retail pharmacies, save when you use an in-network pharmacy and more.
